Release PARTICIPANT AGREEMENT Form

Participants in the studio must sign this Release Form. A participant who is under 18 must have an adult parent/guardian sign this form. A paper copy of the form must be signed by a parent/guardian and sent to the studio. If you are unable to bring in a paper form, please have your parent/guardian download the form online, sign it, then email it or a photo of it to the studio at bellamosaica@hotmail.com before you can participate. Paper copies will be kept on file at the studio for 18 months from last signing.
